Health Hubs in Communities
Your church. Your health. Your community.
Through Health Hubs inside A.M.E. Zion Churches, we connect people to the care they need.
Churches play a big role in many neighborhoods, but they are often left out of the healthcare system. By turning churches into trusted centers of health, we can make our communities stronger and healthier.
Each Health Hub team is trained to teach about health and at least one major disease or condition (like diabetes or high blood pressure).
Every church Health Hub includes:
- Two trusted messengers from the congregation
- One nurse or health coach
- One community pharmacist
Together, the trusted messengers and Health Hub team guide the congregation and community. They share important health information, connect people to care, and act as trusted resources.
At each church, we offer Health Hub services, such as:
- Health education – workshops and resources to learn about staying well
- Health screenings – for example, checking blood pressures
- Health monitoring – ongoing support to track key health needs
- Healthcare coordination – including help with appointments and referrals to doctors, clinics, and pharmacies
Health Hubs link community members with doctors, clinics, and pharmacies, and also provide health coaching as well as assistance with appointments, and telehealth sessions (video or phone visits).
